1. präoperative Vorbereitung
<ol><li>Überprüfen Sie sorgfältig die präoperative Bilder von CT oder MRT für die genaue Lage des Tumors und seine Beziehung zu den umgebenden Geweben.</li>
	<li>Geben Sie dem Patienten eine weiche Kost einen Tag vor der Operation und beginnen um Mitternacht vor der Operation zu fasten.</li>
	<li>Legen Sie den Patienten auf dem OP-Tisch. Dann legen Sie einen peripheren Venenkatheter und induzieren Sie Vollnarkose mit Standardverfahren. Stellen Sie einen zentralen Venenkatheter durch den richtigen internen Halsschlagader für Medikamente Flüssigkeiten und eine periphere arterielle Katheter für Blutdruckmessgerät.</li>
</ol>(2) Position und Port Platzierung des Patienten
<ol><li>Legen Sie den Patienten in Rückenlage und 20° reverse Trendelenburg-Position mit dem Beine Split. In der normalen sterilen Mode für obere Bauchchirurgie drapieren.</li>
	<li>Position der Chirurg-Konsole auf der linken Seite des Patienten. Legen Sie den Patienten Wagen über den Kopf des Patienten und der Vision-Wagen auf der rechten Seite des Patienten, bzw.. Haben Sie der Chirurg zwischen die Beine des Patienten zur Durchführung der Verfahren stehen. Positionieren Sie die rückseitige Tabelle für die Instrumente und Zubehör (siehe Tabelle der Materialien) an der linken Rückseite des Assistenten.</li>
	<li>Machen Sie einen 1 cm Schnitt mit dem Skalpell 3 cm minderwertig und rechts seitlich um den Nabel. Einfügen einer Veress-Nadel in die Bauchhöhle durch diesen Einschnitt und eine CO2 Pneumoperitoneum 14 mmHg mit dem automatischen Insufflation Instrument zu etablieren.</li>
	<li>Entfernen Sie die Veress-Nadel und fügen Sie dann eine 12-mm-Trokar durch 1 cm Schnitt als die Kamera über Port. Legen Sie das Roboter-Endoskop und führen eine diagnostische Laparoskopie (muss der Chirurg die Roboter Endoskop zu halten) den Bauch Adhäsion Status und operativen Machbarkeit bestätigen. Fügen Sie die restlichen 4 Trokare wie folgt unter dem Blick des Endoskops.</li>
	<li>Legen Sie ein 8-mm-Trokar in der linken vorderen axillarlinie (Abbildung 2) auf der Ebene der Nabel für den ersten Roboterarm. Legen Sie eine 12-mm-Trokar 2 cm minderwertig und linke seitliche um den Nabel (Abbildung 2) als Assistent Port.</li>
	<li>Legen Sie eine 12-mm-Trokar in die richtige Midclavicular Linie (Abbildung 2) auf der Ebene des Nabels. Legen Sie ein 8-mm-Trokar in dieser 12-mm-Trokar "Trokar in Trokar" Weise für den zweiten Roboterarm. Platzieren Sie ein 8-mm-Trokar unter den kostalen Seitenrand in der rechten mittleren axillarlinie für den dritten Roboterarm (Abbildung 2). Nach dem Andocken des Roboter-Armee dock das Roboter-Endoskop in den Kameraarm.</li>
</ol>(3) Mobilisierung der Bauchspeicheldrüse Hals und Körper
<ol><li>Greifen und halten erhebt der vorderen Wand des Magens durch Zange auf die dritte Roboterarm, das Gastro-Ligament verfügbar zu machen.</li>
	<li>Spannen Sie das Gastro-Ligament mit bipolaren Pinzette in die zweite Roboterarm und die habgierigen Zange in der Hand des Helfers. Teilen Sie das Gastro-Ligament um geben die geringeren Sac und setzen die Vorderfläche der Bauchspeicheldrüse mit der laparoskopischen Ultraschall-Skalpell auf die erste Roboterarm. Führen Sie Blutstillung mit bipolaren Pinzette auf die zweite Roboterarm bis zum Erreichen der Stufe der richtige Gastroepiploic Vene.</li>

Ausgehend von der unteren zur überlegenen Richtung, teilen der hinteren Wand des Pankreas Halses aus der Pfortader (PV), überlegenen mesenterialen Vene (SMV), minderwertigen mesenterialen Vene (IMV) und Milz Vene (SV) mit dem Cautery Haken. Danach erstellen Sie einen Tunnel zwischen der PV-SMV-IMV-SV und der hinteren Wand des Halses Pankreas.</li>
	<li>Sezieren Sie die Bauchspeicheldrüsen Körper aus der Milz Gefäße und Bindegewebe nach Pankreas hinten mit Cautery Haken und Ultraschall-Skalpell.</li>
</ol>4. die Durchtrennung des Pankreas Parenchym
<ol><li>Legen Sie die laparoskopische Ultraschallsonde durch den Assistenten Trokar und führen Sie die Sonographie an der Bauchspeicheldrüse, um die Position und Größe der Läsion zu bestätigen. Markieren Sie nach dem Ergebnis der Sonographie zwei Durchtrennung Linien in ca. 1 cm weg von der Läsion auf der pankreatischen Oberfläche mit dem Cautery Haken.</li>
	<li>Heben Sie die Bauchspeicheldrüsen Körper mit der Zange durch den minderwertigen Rand der Bauchspeicheldrüse einfügen und weiter nehmen Sie den proximalen Teil des Pankreas Körper aus der Milz Gefäße und Bindegewebe mit der Cautery Haken oder Ultraschall-Skalpell ab.</li>
	<li>Nach Mobilisierung der Bauchspeicheldrüse Hals und proximalen Bauchspeicheldrüsen-Körper zu bilden Sie, die hinteren Gefäße und Gewebe, einzuschneiden Sie das Pankreas Parenchym der distalen und proximalen Durchtrennung entlang mit dem Ultraschall-Skalpell und setzen Sie den Pankreasgang. Dann Transekt das Parenchym rund um den Pankreasgang.</li>
	<li>Sorgfältig zu schützen und den Pankreasgang aus der durchtrennten Pankreas Parenchym zu mobilisieren. Dann scharf Transekt Pankreasgang ca. 1 cm vom Stumpf, mit einer laparoskopischen Schere vom Assistent Hafen.</li>
</ol>5. Rekonstruktion der pankreatischen Kontinuität
<ol><li>Um die Spannung der Anastomose zu verringern, weiter mobilisieren des pankreatischen stumpfes aus dem hinteren Gefäße und Bindegewebe, unter Verwendung der Cautery Haken und bipolare Pinzetten.</li>
	<li>Wählen Sie anhand der Größe der Pankreasgang Stümpfe, einen 10 cm Kunststoff Pankreas Stent mit einem richtigen Durchmesser (1,2 mm Durchmesser in diesem Fall). Schneiden Sie beide Enden des Stents auf schrägen Ebenen und machen Sie mehrere Seite Perforationen zu. Den Stent in die Bauchhöhle über den Assistent Port Implantat.</li>
	<li>Die Seitenwand der Pankreasgang stumpf im Pankreas Körper mit Mikro Zange halten und den Stent gefangen von der Nadel-Fahrer in den distalen Pankreasgang stumpf vorsichtig einsetzen. Naht der Pankreasgang mit der Stent mit 5: 0 resorbierbaren Naht, so dass der Stent eng von den Pankreasgang umgeben ist.</li>
	<li>Stecken Sie das andere Ende des Stents in den proximalen Pankreasgang stumpf mit einem ähnlichen Ansatz aber ohne Naht.</li>
	<li>Naht der proximalen und distalen Pankreas Stümpfe mit horizontalen Matratze Naht (4: 0 nicht resorbierbaren Naht).</li>
	<li>Ziehen Sie die beiden Pankreas Stümpfe näher, und weiterhin das proximale Ende des Stents in den proximalen Pankreasgang stumpf einlegen. Schließlich halten die ganze Stent in den Pankreasgang und bringe es nicht in das Lumen der Zwölffingerdarm.</li>

	<li>Verknoten Sie die restlichen Verbindungen auf die Bauchspeicheldrüse Stümpfe.</li>
	<li>Naht des vorderen Teils der Bauchspeicheldrüse Stümpfe mit kontinuierlicher Naht mit 4: 0 nicht resorbierbarem Nahtmaterial (Abbildung 3).</li>
</ol>6. Blutstillung und Entwässerung
<ol><li>Sorgfältig überprüfen Sie Blutungen Websites und durchzuführen Sie gründliche Blutstillung. Umschließen Sie die Anastomose Website mit resorbierbare blutstillende Gaze.</li>
	<li>Resezierten Exemplar in die Plastiktüte und entfernen Sie den Beutel aus einer erweiterten Einschnitt in den Kamera-Anschluss.</li>
	<li>Legen Sie zwei Abflüsse entlang der oberen und unteren Grenze des Standortes Anastomose und extrahieren Sie die Abflüsse aus dem Hafen für den dritten Roboterarm zu.</li>
</ol>7. Posteoperative Pflege
<ol><li>Nach der Operation zu geben, dem Patienten intravenös Antibiotika (Ceftriaxon Natrium), parenterale Ernährung (Glukose, zusammengesetzte Vitamin zusammengesetzte Aminosäure, Fett-Emulsion, Insulin, KCl und NaCl), Schmerzmittel (Sufentanil, Ondansetron), Somatostatin, Protonenpumpe Inhibitoren (Lansoprazol) und andere Behandlung als identisch mit dem herkömmlichen zentralen Pancreaticectomy. Halten Sie den Patienten Fasten, dann entfernen Sie die nasogastrale und ernähren Sie eine klare flüssige Ernährung sich von den 2Nd postoperativen Tag. Schrittweise Übergang zur normalen Ernährung, wenn der Patient es tolerieren kann.</li>
	<li>Testen Sie die Abfluss-Flüssigkeit für die Amylase und bakterielle Kultur nach 3rd postoperativen Tag. Wenn es keinen Beweis von Pankreas Fisteln und Infektionen gibt und der Abfluss weniger als 10 mL/Tag beträgt, entfernen Sie die Kanalisation.</li>

Bösartige Tumore befinden sich in den Hals der Bauchspeicheldrüse und der proximalen Teil des Pankreas Körpers sind oft mit pankreatikoduodenektomie oder distale pankreatektomie behandelt. Jedoch für benigne oder niedrig maligne mögliche Tumoren in diesen Seiten, wird die standard pankreatikoduodenektomie oder distale pankreatektomie übermäßige normale Gewebe zu entfernen und übermäßige Verletzungen verursachen. Daher wird der zentrale pankreatektomie oder Pankreas Enukleation, bekannt als "Parenchym-schonende pankreatektomie", eine bevorzugte Wahl für die Behandlung von gutartigen und niedrigen potenziellen Malignomen in der Bauchspeicheldrüse Hals und Körper.
Bereits 1908 hatte Ehrhardt der zentralen pankreatektomie15berichtet. Derzeit die am häufigsten verwendeten Rekonstruktionen für Pancreatico-Darm Kontinuität nach zentralen pankreatektomie sind wie folgt: (1) über Nähen des proximalen Pankreas Überrest und Durchführung von Pancreaticojejunostomy für das distale Pankreas Überbleibsel; (2) über Nähen des proximalen Pankreas Überrest und Durchführung von Pancreaticogastrostomy für die distale Pankreas Überrest. Diese zwei Wiederaufbau Ansätze gewährleisten eine entsprechende Spannung für Anastomose. Aber sie stören die anatomische Kontinuität der Verdauungstrakt und ändert die exokrinen Weg der Bauchspeicheldrüse. Da die Pankreas-Enzyme durch intestinale Saft in die Pancreaticojejunostomy aktiviert werden konnte, die Möglichkeit der Klasse C Pankreas Fistel ist relativ höher im zentralen pankreatektomie, so dass die kurzfristige Komplikationen wie Blutungen auch 8,16zu erhöhen. Darüber, wie der Pankreatische stumpf direkt Magensaft in die Pancreaticogastrostomy kontaktiert, könnte Enzyme, so inaktiviert werden theoretisch bessernde die Erosion der Anastomose. Aber es könnte auch führen, exokrine Pankreasinsuffizienz, und auf lange Sicht gefährden den Patienten Ernährungszustand9,10.
Als eine selten verwendete Ansatz könnte die End-to-End-Anastomose der pankreatischen Stümpfe die Nachteile des vorgenannten Wiederaufbau Ansatzes beheben; Es könnte braucht nicht die Pancreaticoenteric Anastomose und die Integrität und Kontinuität des Verdauungstraktes. Es könnte auch halten, der Pankreassaft Sekretion von seiner ursprünglichen physiologischen Struktur. Jedoch vielleicht wegen seiner technischen Komplexität, nur ein paar der offenen zentralen pankreatektomie mit End-to-End-Anastomose Fälle, und dieser Ansatz nicht nacheinander angewendet wurde17,18,19, 20.
Als der am weitesten fortgeschrittene Vertreter der minimal-invasiven Chirurgie chirurgischen Robotersystem wurde in der zentralen pankreatektomie angewendet, und die aktuellen Wiederaufbau Ansätze sind zu nähen den proximalen Pankreas Überrest und Durchführung Pancreaticojejunostomy oder Pancreaticogastrostomy für die distale Pankreas Überbleibsel6,21,22,23. Unser op-Team hat reiche Erfahrung in Roboter pankreatikoduodenektomie, zentrale pankreatektomie und distale pankreatektomie13,24. Wir haben auch Roboter Pankreas Enukleation mit wichtigsten Pankreasgang Invasion und Verletzungen durchgeführt. In dieser Operation repariert wir die Pankreasgang Verletzungen durch Einfügen eines Pankreas Stents und mit End-to-End-Anastomose14. Basierend auf unserer Erfahrung im Pankreas roboterchirurgie, haben wir die traditionellen Roboter zentrale pankreatektomie mit End-to-End-Anastomose, maximieren Sie das verbleibende Pankreas Parenchym und die Verletzung von Patienten hochgelagert geändert.
Derzeit haben wir chirurgischen Robotersystem führen die zentrale pankreatektomie und rekonstruieren die Bauchspeicheldrüse Kontinuität durch den Pankreasgang Plastik und Bauchspeicheldrüse End-to-End-Anastomose. Wir fanden, dass die Kurzzeit-Ergebnisse günstig waren. Wie der chirurgischen Robotersystem ein Vergrößerungsglas 3D-Ansicht, Multi-Freiheitsgrad für die Bewegung der Instrumente, die Beseitigung der physiologischen Vorhofflimmern und andere Eigenschaften hat zeigt dieses chirurgische System inhärenten Vorteile in der Dissektion, Plastik und Anastomose der Pankreasgang. Verglichen mit konventionellen Wiederaufbau Ansätze, diesen Ansatz vermeidet die Pancreaticoenteric Anastomose des Pankreas distalen stumpfes, verkürzt die Operationszeit und reduziert die Verletzung. Die direkte Anastomose der Bauchspeicheldrüse zwei Stümpfe vermeidet den pankreatischen stumpf Kontakt Verdauungssäften, die theoretisch das Risiko von Bauchspeicheldrüsenkrebs Fistel und Blutungen reduzieren könnte. Die Platzierung des Stents ermöglicht auch die Anastomose der Pankreasgang, sicherer und schneller zu sein. Darüber hinaus verhindert kontinuierliche Unterstützung durch den Stent auch die postoperative Striktur der Pankreasgang. Auf der anderen Seite auch wenn Pankreas Fistel aufgetreten ist, könnte die Bauchspeicheldrüse Flüssigkeit in den Zwölffingerdarm durch Stent anstelle fließt außerhalb der Bauchspeicheldrüse, wodurch sich die Schäden der Bauchspeicheldrüse Fistel zu den umliegenden Organen fließen. Da der Stent zu den Pankreasgang mit resorbierbarem Nahtmaterial vernäht ist, wird der Stent spontan innerhalb von ein paar Monaten abfallen.
Der entscheidende Schritt des Verfahrens ist die Anastomose der Pankreasgang und Parenchym ohne Spannung. Um eine spannungsfreie Anastomose zu erreichen, sollten die Bauchspeicheldrüse Stümpfe ausreichend von ihren hinteren Gefäße und Gewebe, besonders der distalen stumpf mobilisiert werden. Es sei darauf hingewiesen, dass die Durchtrennung der Pankreasgang durch Schere anstatt Energie Instruments, durchgeführt wurde, reduzieren den Schaden von den Pankreasgang und ermöglichen eine bessere Rohr zu Rohr-Anastomose. Darüber hinaus sollte die Stümpfe der Pankreasgang aus der resezierten Bauchspeicheldrüse für Plastik so weit wie möglich zerlegt werden. Der pankreatischen Stent wir in der Regel sterilisieren Kunststoff-Rohre mit unterschiedlichen Durchmessern und Längen durch das Operation Center und wählen Sie die richtige Größe während des Betriebs.
Wir fanden, dass die Roboter zentrale pankreatektomie mit End-to-End-Anastomose bei ausgewählten Patienten sicher und machbar ist. Das chirurgische Vorgehen reduziert den Umfang der zentralen pankreatektomie und bewahrt die Integrität und Kontinuität der Physiologie und Anatomie, die die minimal-invasive Merkmale der chirurgischen Robotersystem in pankreasoperation widerspiegelt. Da diese Operation in einer ersten Phase ist, sind viele Fragen noch unklar, wie rational Somatostatin postoperativ verwenden; für Patienten mit langen Defekt der Pankreasgang sollten wir ob zentrale pankreatektomie wählen; Wenn der Stent nicht fallen spontan deaktiviert ist, ist was die Nebenwirkung, etc. bleiben seine technische Änderung und langfristige Wirksamkeit auch erforscht werden.

Zentrale pankreatektomie wird zunehmend für die Behandlung von gutartigen oder niedrig-maligne potenzielle Tumoren befindet sich in der Bauchspeicheldrüse Hals oder proximalen Teil des Pankreas Körper1durchgeführt. Verglichen mit der Duodenopankreatektomie oder distale pankreatektomie, zentrale pankreatektomie resects weniger Gewebe und spart mehr Pankreas Parenchym und Funktion. Derzeit sind die wichtigsten Ansätze für den Wiederaufbau der Bauchspeicheldrüse übermäßig nähen den cephalic Pankreas stumpf und Ausführen eines Pancreaticojejunostomy oder Pancreaticogastrostomy der distalen stumpf2,3. Diese beiden Ansätze sind weit verbreitet in der offenen, laparoskopischen und Roboter-zentrale Pancreatectomies4,5,6,7. Allerdings brechen die oben genannten zwei Wiederaufbau Ansätze die anatomische Kontinuität der Verdauungstrakt und der Weg für die Ausscheidung der Bauchspeicheldrüse Flüssigkeit. Den direkten Kontakt des Pankreas stumpf mit Darm Saft könnte die Möglichkeit von Blutungen und Fistel8erhöhen. Obwohl den Magensaft inaktiven Pankreasenzyme um die Erosion der Anastomose zu vermeiden könnte, es könnte führen zu exokrinen Pankreasinsuffizienz und auf lange Sicht gefährden den Patienten Ernährungszustand9,10.
Mit der Entwicklung der minimal invasiven Chirurgie, Roboter-Chirurgie hat große Vorteile in seiner Lupe 3D-Ansicht, Stabilität, und erhöht die Flexibilität der Bewegungen, etc., die bietet sezieren und nähen Kapazität für Anastomose und Blutstillung11,12. Basierend auf unserer Erfahrung in Roboter pankreatektomie und Reparatur für verletzte Pankreasgang13,14, haben wir eine Reihe von Roboter-zentrale pankreatektomie mit End-to-End-Anastomose durchgeführt und günstige Ergebnisse zu sehen. Verglichen mit Pancreaticojejunostomy oder Pancreaticogastrostomy, vermeidet Bauchspeicheldrüse End-to-End-Anastomose Schäden an Verdauungstrakt; wodurch theoretisch die Möglichkeit einer Pancreaticoenteric Fistel. Aber auf der anderen Seite stellt die Bauchspeicheldrüse End-to-End-Anastomose größere technische Schwierigkeiten. Als Update der herkömmlichen zentralen pankreatektomie sind die Kandidaten für die konventionelle zentrale pankreatektomie ebenfalls förderfähig für diese Operation. Hier präsentieren wir Operationstechniken für Roboter-zentrale pankreatektomie mit End-to-End-Anastomose in einem tertiären Hepatopancreatobiliary Center in diesem Fall Videopräsentation.
Indikationen:
(1) Benin und niedrig-malignen mögliche Tumoren befindet sich in der Bauchspeicheldrüse Hals und proximalen Körper eignen sich für diesen Vorgang. (2) der Mangel der wichtigsten Pankreasgang sollte weniger als 5 cm sein, nachdem die zentrale pankreatektomie und die Größe des Tumors ist nicht das Hauptanliegen für den Wiederaufbau.
Fall Präsentation:
Der Patient ist ein 31 Jahre alter Mann. Eine Läsion in der Bauchspeicheldrüse Hals wurde versehentlich in eine ärztliche Untersuchung vor 2 Jahren gefunden. Er unterzog sich regelmäßige Überprüfung und die Läsion wurde festgestellt, dass vor kurzem asymptomatisch vergrößert werden. Er hatte keine Geschichte der früheren Bauchoperationen. Bei der körperlichen Untersuchung gab es kein positiven Zeichen. Das Pankreas MRI zeigte eine quasi-kreisförmige Läsion ca. 14mm in der Bauchspeicheldrüse Hals; Dies wurde als ein neuroendokriner Tumor vermutet. Auf T1 - und T2-gewichtete Bildgebung, signalisieren die Läsion zeigte Hypointense und hyperintens, beziehungsweise. Auf die diffusionsgewichtete Bildgebung zeigte die Läsion hyperintens Signal. Die Läsion wurde leicht verbessert in der arteriellen Phase und zeigten eine progressive Enhancement-Muster in das Parenchym Phase und Verzögerung Phase (Abbildung 1). Schwerer Fettleber wurde auch durch die MRT diagnostiziert. Keine Dilatation oder Striktur war in den Pankreasgang erkannt. Routine-Blutuntersuchung, IgG-Isoform Test Tumor Biomarker, EKG und Röntgen waren normal. Die biochemische Test zeigte leicht erhöhte ALT (Alanin-Aminotransferase) bei 147,3 U/L und AST (Aspartat-Aminotransferase) bei 53.9U / L, die durch Fettleber verursacht werden können.

application of end-to-end anastomosis in robotic central pancreatectomy

Zentrale pankreatektomie wird zur Behandlung von gutartigen oder niedrig-maligne potenzielle Tumoren befindet sich in der Bauchspeicheldrüse Hals oder proximalen Teil des Pankreas Körper durchgeführt. Mit der technologischen Entwicklung hat chirurgischen Robotersystem seinen Vorteil in der minimal-invasiven Chirurgie gezeigt und wurde zunehmend in zentralen pankreatektomie angewendet. Wiederaufbau der Kontinuität der Bauchspeicheldrüse mit End-to-End-Anastomose nach Roboter zentrale pankreatektomie wurde jedoch nicht angewandt. In dieser Studie berichten wir Operationstechniken für Roboter-zentrale pankreatektomie mit End-to-End-Anastomose. Die Bauchspeicheldrüse wird von Rohr zu Rohr Anastomose der Pankreasgang mit einem Pankreas Stent in die beiden Stümpfe der Pankreasgang eingefügt und von End-to-End-Anastomose der Pankreas Parenchym rekonstruiert. Verglichen mit traditionellen zentralen pankreatektomie mit Pancreaticoenteric Anastomose, dieser Ansatz verringert die operative Verletzungen des Patienten und auch bewahrt die Integrität und Kontinuität der Verdauungs-Kanal und Pankreasgang. Die chirurgischen Robotersystem integriert mit mehreren Instrumenten mit flexiblen und präzisen Bewegung eignet sich besonders für die Dissektion und Rekonstruktion der Pankreasgang. Wir fanden, dass Roboter zentrale pankreatektomie mit End-to-End-Anastomose sicher und machbar ist, und wir mehr Erfahrung brauchen, um ihre besten Indikationen und Langzeitergebnisse bewertet.

Der Vorgang verlief glatt. Die Operationszeit 141 Minuten und der intraoperativen Blutverlust war ca. 50 mL. Die Größe des resezierten Pankreas war ca. 5,5 × 2,5 × 2,5 cm (Abbildung 4). Der Patient erholte sich reibungslos, nach der Operation und auf die 6th postoperativen Tag entlassen. Wenn der Patient entlassen wurde, er hatte eine normale Ernährung und normalen Stuhlgang, und hatte kein Fieber, keine Bauchschmerzen oder Blähungen. Die routinemäßige Untersuchung und Blut biochemische Blutuntersuchung hatte keine offensichtliche Abnormität. Postopertative CT-Scan zeigte leichte Exsudation um die Bauchspeicheldrüse und keine Dilatation oder Striktur in den Pankreasgang (Abbildung 5). Die abdominal Abfluss lag ca. 100 mL/Tag. Die pathologische Untersuchung bestätigt einen solide vasoaktive Tumor mit negativen Marge und der Immunhistochemie-Test zeigte, dass der Tumor B-Catenin (Kern und Kapsel +), PR (+ 60 %), Vimentin (+), Cga (-), CD10 (+), Syn (+), CK (fokale +), CD56 (+), Ki-67 (+ 2 %). Die Abflüsse waren auf der 20th postoperativen Tag entfernt.

Murine Ileocolic Bowel Resection with Primary Anastomosis

Intestinal resections are frequently required for treatment of diseases involving the gastrointestinal tract, with Crohn&#8217;s disease and colon cancer being two common examples. Despite the frequency of these procedures, a significant knowledge gap remains in describing the inherent effects of intestinal resection on host physiology and disease pathophysiology. This article provides detailed instructions for an ileocolic resection with primary end-to-end anastomosis in mice, as well as essential aspects of peri-operative care to maximize post-operative success. When followed closely, this procedure yields a 95% long-term survival rate, no failure to thrive, and minimizes post-operative complications of bowel obstruction and anastomotic leak. The technical challenges of performing the procedure in mice are a barrier to its wide spread use in research. The skills described in this article can be acquired without previous surgical experience. Once mastered, the murine ileocolic resection procedure will provide a reproducible tool for studying the effects of intestinal resection in models of human disease.

Ileocolic resection is commonly performed in several human diseases; however, little is known regarding the impact of intestinal resection on surgical illnesses. This article provides instruction on executing the procedure in mice with high success, providing a means to study the effects of ileocolic resection in models of disease.

Robotic Ablation of Atrial Fibrillation

Background: Pulmonary vein isolation (PVI) is an established treatment for atrial fibrillation (AF). During PVI an electrical conduction block between pulmonary vein (PV) and left atrium (LA) is created. This conduction block prevents AF, which is triggered by irregular electric activity originating from the PV. However, transmural atrial lesions are required which can be challenging. Re-conduction and AF recurrence occur in 20 - 40% of the cases. Robotic catheter systems aim to improve catheter steerability. Here, a procedure with a new remote catheter system (RCS), is presented. Objective of this article is to show feasibility of robotic AF ablation with a novel system. Materials and Methods: After interatrial trans-septal puncture is performed using a long sheath and needle under fluoroscopic guidance. The needle is removed and a guide wire is placed in the left superior PV. Then an ablation catheter is positioned in the LA, using the sheath and wire as guide to the LA. LA angiography is performed over the sheath. A circular mapping catheter is positioned via the long sheath into the LA and a three-dimensional (3-D) anatomical reconstruction of the LA is performed. The handle of the ablation catheter is positioned in the robotic arm of the Amigo system and the ablation procedure begins. During the ablation procedure, the operator manipulates the ablation catheter via the robotic arm with the use of a remote control. The ablation is performed by creating point-by-point lesions around the left and right PV ostia. Contact force is measured at the catheter tip to provide feedback of catheter-tissue contact. Conduction block is confirmed by recording the PV potentials on the circular mapping catheter and by pacing maneuvers. The operator stays out of the radiationfield during ablation. Conclusion: The novel catheter system allows ablation with high stability on low operator fluoroscopy exposure.

Emergency Undocking in Robotic Surgery: A Simulation Curriculum

The following is a training platform to allow robotic surgeons to develop the skills necessary to lead an interprofessional team in emergency undocking of a robotic system. In traditional robotic training for surgeons, a brief web based overview of performing an emergency undocking is provided during initial introductory training to the robotics system. During such a process, there is no training in delineation of interdisciplinary roles for operating room (OR) personnel. The training presented here uses formative simulation and debriefing followed by a lecture. For the simulation, a modified gynecologic simulator is draped in a steep trendelenburg position consistent with most gynecologic laparoscopic surgery. The training torso is modified using tubing hooked to pressure bags of red food colored IV fluid used to simulate a catastrophic vessel injury on demand. Positioned throughout the operating room setting is an interprofessional team consisting of embedded standardized persons (ESPs) to fulfill the roles of a circulating nurse, scrub nurse, anesthesiologist, and bedside assist surgeon. Robotic surgeons are presented a case scenario necessitating emergency undocking, and given control of the robotic instruments. The scenario is terminated following either successful completion of an emergency undock, or at five minutes due to the emergent nature of the case. A debriefing session with hands on training of the steps to emergency undocking, necessary equipment, troubleshooting techniques, and operating room personnel roles follows the simulation. The learners are presented with a short lecture reemphasizing the material presented in the debriefing for their own self-study. This training results in improved time accessing the patient, improved knowledge, confidence, and completion of critical actions, and can be replicated in most institutions. All robotic surgeons should be able to demonstrate competence in this crucial intervention. A limitation of the curriculum is ability to access the in-situ environment for training purposes.

This training platform is designed to allow robotic surgeons to develop the skills necessary to lead an interprofessional team in emergency undocking of the robotic system. Training includes the utilization of the technology and equipment to perform an emergency undocking, as well as delineation of roles for such a scenario.

Vessel-sparing Excision and Primary Anastomosis

Urethroplasty is considered to be the standard treatment for urethral strictures since it provides excellent long-term success rates. For isolated short bulbar or posterior urethral strictures, urethroplasty by excision and primary anastomosis (EPA) is recommended. As EPA only requires the excision of the narrowed segment and the surrounding spongiofibrosis, a full-thickness transection of the corpus spongiosum, as performed in the traditional transecting EPA (tEPA), is usually unnecessary. Jordan et al. introduced the idea of a vessel-sparing approach in 2007, aiming to reduce surgical trauma, especially to the dual arterial blood supply of the urethra, and, thus, potentially reducing the risk of postoperative erectile dysfunction or glans ischemia. This approach could also be beneficial for subsequent urethral interventions such as redo urethroplasty using a free graft, in which a well-vascularized graft bed is imperative. Nevertheless, these potential benefits are only assumptions as prospective studies comparing the functional outcome of both techniques with validated questionnaires are currently lacking. Moreover, vessel-sparing EPA (vsEPA) should at least be able to provide similar surgical outcomes as tEPA. The aim of this paper is to give an elaborate, step-by-step overview of how to manage patients with isolated short bulbar or posterior urethral strictures with vsEPA. The main objective of this manuscript is to outline the surgical technique and to report the representative surgical outcome. A total of 117 patients were managed according to the described protocol. The analysis was performed on the entire patient cohort and on the bulbar (n = 91) and posterior (n = 26) vsEPA group separately. Success rates were 93.4% and 88.5% for the bulbar and posterior vsEPA, respectively. To conclude, vsEPA, as outlined in the protocol, provides excellent success rates with low complication rates for isolated short bulbar and posterior urethral strictures.

Creation of Colonic Anastomosis in Mice

Intestinal anastomoses are commonly performed in both elective and emergent operations. Even so, anastomotic leaks are a highly feared complications of colonic surgeries and can occur in up to 26% of surgical anastomoses, with mortality being up to 39% for patients with such a leak. Currently, there remains a paucity of data detailing the cellular mechanisms of anastomotic healing. Devising preventative strategies and treatment modalities for anastomotic leak could be greatly potentiated by a better understanding of appropriate anastomotic healing.
A murine model is ideal as previous studies have shown that the murine anastomosis is the most clinically similar to the human case as compared with other animal models. We offer an easily reproducible murine model of colonic anastomosis in mice that will allow for further illustration of anastomotic healing.

Anastomotic leak or breakdown after surgery is a major cause of postoperative morbidity and mortality. Our surgery for creating a colonic anastomosis is a reliable and reproducible method for studying the healing mechanisms of anastomoses.

One-anastomosis Gastric Bypass (OAGB) in Rats

The goal of this protocol is to set up a preclinical model of bariatric surgery and, more specifically, OAGB in obese rats. Based on this preclinical model, longitudinal studies can be carried out to provide an improved understanding of the mechanisms underlying the outcomes seen after bariatric surgery in humans. For this purpose, rats are operated on through a laparotomy under general anesthesia with isoflurane. First, the surgeon creates a long and tubular gastric pouch: after greater curve and hiatal dissection, the nonglandular stomach is stapled and removed. Then, the remaining stomach is also stapled in order to create a gastric tube and exclude the antrum of the stomach. After that, the surgeon performs a single end-to-side gastrojejunostomy 35 cm from the duodenojejunal angle. This limb length has been chosen in order to reproduce the same ratio between the biliopancreatic limb (BPL) and common limb (CL) length as in human bariatric surgery. The operation ends by aponeurotic and cutaneous closure. The early postoperative management consists of subcutaneous hydration, an intramuscular prophylactic antibiotic injection, a parietal injection of xylocaine, the administration of painkillers, and a progressive reintroduction of diet.

DIPLOMA Approach for Standardized Pathology Assessment of Distal Pancreatectomy Specimens

Pancreatic ductal adenocarcinoma (PDAC) is one of the most lethal malignant cancers. A minority (20%) of PDACs are found in the pancreatic body and tail. Accurate pathology assessment of the pancreatic specimen is essential for providing prognostic information and it may guide further treatment strategies. The recent 8th edition of the American Joint Committee on Cancer/Union for International Cancer Control (AJCC/UICC) staging system for pancreatic tumors has incorporated significant changes to tumor (pT) stage, which is predominantly based on tumor size. This change emphasizes the importance of careful block selection. Owing to the greater prevalence of tumors in the head of the pancreas, efforts are made to standardize the assessment of pancreatoduodenectomy specimens. However, consensus regarding the macroscopic assessment of distal (i.e., left) pancreatectomy specimens is lacking. The DIPLOMA approach includes the standardized measurement of pancreas and other resected organs, inking of relevant surgical margins and anatomical surfaces without removing covering layers of fat, measurement of tumor size (for T-stage), together with assessment of splenic vessel involvement (and other organs if present). All relevant margins are assessed, and relevant blocks are selected to confirm these parameters microscopically. The current protocol describes a standardized approach to the macroscopic assessment of distal pancreatectomy specimens. This approach was developed during several meetings with pathologists and surgeons during the preparation phase for an international multicenter trial (DIPLOMA, ISRCTN44897265), which focuses on radicality of distal pancreatectomy for pancreatic ductal adenocarcinoma. This standardized approach can be instrumental in the design of studies and will uniform reporting on the outcomes of distal pancreatectomy. The described technique is used in the DIPLOMA trial for pancreatic ductal adenocarcinoma but may also be useful for other indications.

The current study highlights a standardized approach to the macroscopic assessment of distal pancreatectomy specimens for pancreatic ductal adenocarcinoma, with special emphasis on the measurement of pancreatic dimensions and those of other organs, inking of margins, measurement of tumor size and proximity to margins, lymph node sampling and block selection.

Application of Lucilia sericata Larvae in Debridement of Pressure Wounds in Outpatient Settings

Biological therapy using Lucilia sericata larvae has numerous advocates worldwide, yet it is still fairly unknown and not commonly applied in daily practice because of the limited awareness and insufficient experience of medical and nursing personnel. There are case reports suggesting that maggot therapy can be applied and supported by lay caregivers, provided they are supervised and informed by physicians/nurses. The foregoing observation suggests that the method should be considered for implementation by a wider group of caregivers if accepted and meticulously supervised by trained and experienced medical staff. The concerns related to the therapeutic use of maggots in certain regions seem understandable, but are not supported by scientific facts. It should be noted that many therapeutic agents (including brood) used in medicine are of natural origin, and are associated with low production costs and high possibilities of implementation in the course of therapy. By analyzing the literature and using our own clinical and research experience, we have come to conclusions related to using larvae therapy, as a quick and safe method providing cleaning and revitalization in the process of treating wounds of various etiologies, especially pressure ulcers. In the current study, medical-grade Lucilia sericata maggots were applied to remove necrotic tissue from deep pressure sores. The treatment is mostly accepted by both caregivers and patients. In most cases, it is conducted by trained and experienced medical personnel in home and outpatient settings. Over the course of the conducted analyzes involving the collected specimens, there were no statistically significant relationships (p&gt; 0.05) confirmed between the wound surface successfully cleared by brood and variables, such as time from wound formation, location, surface size, and the depth of damage to the tissue structure. The lack of statistical dependence may result from the small size of the studied group. Based on the current findings, we have formulated the following conclusions: Maggot Debridement Therapy (MDT) is a fast and effective method enabling the preparation of the wound bed. The use of MDT in outpatient settings is safe and acceptable for patients and their caregivers.

Robotic Central Pancreatectomy with Roux-en-Y Pancreaticojejunostomy

Central pancreatectomy is a parenchyma-sparing alternative to distal pancreatectomy in patients with a benign or low-grade malignant tumor in the body of the pancreas. The aim of central pancreatectomy is to prevent postoperative life-long endocrine and exocrine insufficiency. The downside of central pancreatectomy is the high rate of postoperative pancreatic fistula, which is the main reason that many surgeons do not routinely use central pancreatectomy in eligible patients. Most studies report open or laparoscopic central pancreatectomy with a pancreatico-gastrostomy anastomosis in adults. This is the first description of a standardized approach to robotic central pancreatectomy with Roux-en-Y pancreaticojejunostomy reconstruction in an adolescent (16-year-old boy) with a pseudopapillary tumor in the body of the pancreas. The operation time was 248 min with 20 mL of blood loss. The postoperative course was uneventful except for the short-term medical treatment for a grade B pancreatic fistula. Robotic central pancreatectomy can be safely applied in selected patients in experienced centers.

Transoral Robotic Total Thyroidectomy and Bilateral Central Regional Lymph Node Dissection for Papillary Thyroid Carcinoma

Currently, the common approaches to thyroid surgery include conventional thyroidectomy, bilateral axillo-breast, axillary, retroauricular, and oral vestibule approaches. Essentially, various approaches to thyroid surgery only move the traditional surgical incision to a more concealed position but still leave scars on the body surface. Among them, thyroid surgery via the oral vestibule approach can obtain the best cosmetic outcome through the shortest natural cavity. However, in the early time, thyroid surgery via the oral vestibule approach usually requires three incisions in the mouth and one axillary incision. We have introduced a robotic surgical system into thyroidectomy via the oral vestibule approach and successfully completed total thyroidectomy and bilateral central lymph node dissection. During the operation, only three incisions were made at the oral vestibule without an axillary incision. This article aims to present the unique three-port method of robotic thyroidectomy via oral vestibule for the treatment of patients with papillary thyroid cancer.

The transoral robotic total thyroidectomy and bilateral central regional lymph node dissection using three ports without axillary incision are feasible and safe for early-stage papillary thyroid carcinoma. Here we present the operative techniques of this operation.
