All experiments were approved by the local regulatory agency (LANUV of Nordrhein-Westfalen, Germany) and were performed according to the guidelines of animal use. Animals were fed with a standard chow diet and received tap water ad libitum. All equipment and chemicals necessary to each step of the experiment are available in the Table of Materials.
1. Preparation of the Latex Cap and Intraventricular Balloon
<ol>
	<li>Make an aluminum mold using a milling machine that matches the average size of the rat heart (bodyweight of 300-350 g). Polish the mold with superfine (10/0) emery paper. 
	NOTE: The detailed metrics of the mold are shown in Figure 1A.</li>
	<li>Vertically fix the neck of the aluminum mold to the milling machine to prepare the latex cap. 
	NOTE: The milling machine causes the mold to slowly rotate. Alternatively, an electric motor can be used.</li>
	<li>Pour 20 mL of liquid latex (commercially purchased, see the Table of Materials) into a 50 mL glass beaker.</li>
	<li>Lower the mold until the entire body of the mold is immersed in the latex solution.</li>
	<li>Slowly lift the mold (5 cm/min) while rotating.</li>
	<li>Keep rotating the mold for an additional 15 min, until the latex on the surface of the mold is solidified.</li>
	<li>Add about 1 g of talcum powder to the surface of the mold (already covered by a thin latex film) to prevent damage while detaching.</li>
	<li>Gently detach with fingers the already dried latex cap from the mold surface; the latex cap is now ready for use (Figure 1B).</li>
	<li>Connect the outlet of the latex cap to 15-cm silicon tubing (ID = 0.2 mm), used later for the collection of CT.</li>
	<li>Fill the ventricular latex balloon with water and firmly fix it onto an L-shaped metal cannula connected to a 1 mL, water-filled syringe (Figure 1C). 
	NOTE: This will be used to ensure the upright positioning of the heart (see below).</li>
	<li>Make sure that the balloon is airtight by performing several deflating/inflating tests with the attached 1 mL syringe.</li>
	<li>Connect the cannula, via a three-way stop, to a pressure transducer for the future measurement of intraventricular developed pressure (Figure 1C).</li>
</ol>
2. Preparation of Krebs-Henseleit Buffer (KHB) and the Langendorff Perfusion System
<ol>
	<li>Set up a Langendorff perfusion system by using either constant-flow (driven by a roller pump) or constant-pressure (generated by static pressure in a glass column) mode. 
	NOTE: Details of the Langendorff heart preparation have been described previously12.</li>
	<li>Prepare 2 L of a modified KHB (in mM: 116.02 NaCl, 4.63 KCl, 1.10 MgSO4&#183;7H2O, 1.21 K2HPO4, 2.52 CaCl2&#183;2H2O, 24.88 NaHCO3, 8.30 D-glucose, and 2.0 sodium pyruvate).
	<ol>
		<li>Weigh all chemicals but CaCl2 and dissolve them in 1.8 L of double-distilled water in a 2-L flask.</li>
		<li>Bubble the medium with carbogen (95% O2/5% CO2) for at least 5 min for equilibration (pH: 7.4) under magnetic stirring.</li>
		<li>Add 0.74 g of CaCl2.2H2O and bring up the total volume to 2 L with double-distilled water.</li>
		<li>Continue stirring and bubbling the medium with carbogen for an additional 5 min.</li>
		<li>Filter the KHB through a 0.2 &#181;m filter to eliminate small particles that may obstruct the microcirculation of the heart.</li>
	</ol>
	</li>
	<li>Preparation of the Langendorff perfusion system.
	<ol>
		<li>Place the filtered KHB into a pre-warmed water bath (38 &#176;C); keep bubbling with carbogen to generate a pressure of 100 cmH2O inside the KHB reservoir.</li>
		<li>Connect the reservoir to the glass column to establish 100 cmH2O hydrostatic pressure for the Langendorff perfusion with KHB; continue bubbling the KHB inside the column with carbogen.</li>
		<li>Adjust the temperature of the warming system so that the temperature at the aortic cannula outlet is 37 &#176;C.</li>
		<li>Ensure that the tubing system is bubble-free.</li>
		<li>Oxygenate the KHB with carbogen for an additional 5 min, until the PO2 in the KHB reaches 500-600 mmHg (measured by a blood-gas analyzer).</li>
	</ol>
	</li>
	<li>Set up the perfusion system to run either at a constant pressure of 100 cmH2O or at a constant flow of about 10-20 mL/min using manual switching. Alternatively, use an interchangeable STH pump controller to instantly switch to the perfusion mode.</li>
</ol>
3. Isolation and Cannulation of the Heart
NOTE: Male Wistar rats with bodyweights of 300-350 g were used so that the sizes of hearts matched the pre-cast latex cap. Rats underwent either a ligation of the left arterial descending (LAD) for 50 min, followed by reperfusion or were sham operated. Details of the methodology for the induction of myocardial infarction (MI) were reported elsewhere13. The reversed-heart experiments in the infarct animals were performed 5 days after operation.
<ol>
	<li>Anesthetize rats using an isoflurane vaporizer (2% V/V) connected to an animal holding chamber (20 L).</li>
	<li>Transfer the rats to an operation table (not temperature controlled) after deep anesthesia is reached.</li>
	<li>Lift the skin and muscle just below the sternum using forceps and cut around the lower margin of the ribs with heavy scissors.</li>
	<li>Using fine scissors, make a small cut into the diaphragm, at the rib margin. Cut the ribs caudally to make a flap of the entire ventral chest wall.</li>
	<li>Gently grab the heart with the thumb and index and middle fingers and slowly lift it upwards so that the cardiac vessels become slightly stretched.</li>
	<li>Excise the heart until the aorta is fully exposed.</li>
	<li>Place the heart in a 100 mL beaker containing 50 mL of ice-cold KHB (4 &#176;C) and move it to the perfusion apparatus.</li>
	<li>Immediately mount the heart via the aorta onto a dripping cannula and securely tighten it with a suture (4-0). Avoid air bubbles entering the heart.</li>
	<li>Apply constant perfusion pressure (100 cmH2O). Alternatively, a full flow rate (starting with 20 mL/min) can be applied. 
	Note: The time from the opening of the thorax to the attachment of the heart to the perfusion cannula should take about 3 min in the hands of an experienced operator.</li>
</ol>
4. Reversed-heart Model
<ol>
	<li>Gently rotate the aortic cannula until the posterior wall of the heart is in en face view.</li>
	<li>Remove the connective tissue with scissors to expose the opening of the left atrium, making it ready for intraventricular cannulation.</li>
	<li>Insert the deflated latex balloon attached to a rigid catheter via the left atrium into the left ventricle.</li>
	<li>Inflate the balloon until it fills the entire ventricular cavity (the inflating volume is pre-marked on the syringe).</li>
	<li>Invert the heart until it is upside-down, supporting it by the intraventricular balloon catheter.</li>
	<li>As demonstrated in Figure 1C, mechanically stabilize the inverted heart in an upright position using the intraventricular balloon with a rigid metal catheter.</li>
	<li>Adjust the position of the heart to avoid excessive twisting of the aortic root.</li>
	<li>Adjust the diastolic pressure to 3-5 mmHg (measured by the intra-ventricular balloon; see Figure 1C).</li>
	<li>Observe the epicardial surface of the heart and ensure that small droplets form.</li>
	<li>Place the latex cap onto the surface of the heart by gently pushing it to cover the entire heart using the fingers.</li>
	<li>Make sure that the latex cap covers most of the ventricular surface.</li>
	<li>Remove the air bubbles, if any, inside the cap and the tubing by gently sucking with a 1-mL syringe.</li>
	<li>Adjust the distal opening of the CT-letting tubing to 10 cm below the horizontal level of the heart. 
	NOTE: This procedure ensures slight sucking by negative hydrostatic pressure.</li>
	<li>Collect drops of CT in a 1.5 mL collection tube placed in ice mixed 1:1 with NaCl. Collect about 0.15-1.5 mL of CT. 
	NOTE: The ice/NaCl mixture stabilizes the temperature in the collection tube to below zero (about -4 &#176;C). 
	NOTE: The sampling time depends on the experimental purpose. The flow rate of the CT is about 27 &#177; 20 &#181;L/ min in sham-operated animals (n = 3) and 100 &#177; 47 &#181;L/min for the coronary-ligated animals (n = 11).</li>
	<li>Weigh and snap-freeze CT samples in liquid nitrogen and store them at -80 &#176;C for later measurements.</li>
</ol>
5. Analysis of the CT
<ol>
	<li>Use the CT fluid for the analysis of metabolites, depending on the scientific question. 
	NOTE: The data shown in Figure 2 and Figure 3 were collected from a constant-pressure perfusion (100 cmH2O), and about 0.15-1.5 mL of CT fluid was collected within a period of 10 min. This time and volume were sufficient for the proteomic (minimum: 50 &#181;L; Figure 2)14 and HPLC (minimum: 20 &#181;L; Figure 3)15 analysis of various purines.</li>

The reversed-heart model is based on the well-established Langendorff heart perfusion technique12 and is performed by simply inverting the heart into an upside-down position and holding this position using a rigid intra-ventricular balloon catheter. In such a way, cardiac interstitial transudate can be physically separated from coronary venous effluent perfusate, dripping by gravity from the base of the heart9. The CT can be continuously collected by means of a thin and flexible latex cap placed on the surface of the entire heart.
The method is easy to perform, with minimal cost in addition to that of the Langendorff apparatus. Nevertheless, some steps are technically critical for obtaining reproducible and stable results. These include ensuring that the latex cap properly fits the shape of the hearts and covers about 90% of the surface of the ventricles, not the atria. The time from excising the heart from the animal to the performance of the retro-perfusion should be less than 3 min, as prolonged ischemia bears the risk of altering cardiac metabolism and the formation of cardiac transudate. The diastolic left ventricular pressure, measured by the intra-ventricular balloon, should be set to fill the ventricular cavity (3-5 mmHg). An over-inflated balloon may alter coronary flow by vascular compression. The sampling vial (1.5 mL tube) should be kept on ice to avoid any potential degradation of metabolites and proteins of interest.
Additionally, successful experiments are critically dependent upon good manual handling during the preparation, isolation, and cannulation of the heart. This requires practice. To protect hearts from ischemic damage, all preparations should be carried out with ice-cold KHB. Since the sizes of the hearts may vary between rats, in spite of similar bodyweights, it is advisable to have latex caps prepared with slightly different dimensions that suit the different sizes of the hearts.
The isolated reversed-heart method has been previously described for the isolated rat5,6,7,8,9,10 and guinea pig heart16 and was used for different purposes. In our present description of the methodology, we have introduced some modifications to the experimental setup and to sample processing. For example, the cannulation of the pulmonary artery, as introduced by de Deckere et al.7, was not performed here, since the inverted position of the heart prevents potential contamination with venous effluent perfusate. The collecting device for CT was simplified by introducing a slight negative pressure by lowering the opening of the CT-letting tubing to 10 cm beneath the reverse heart. This makes it easier to immediately cool down the transudate samples. To ensure the rapid cooling of the sample CT, the collection cups were precooled to a temperature of -4 &#176;C by placing them into an equal-volume mixture of ice and NaCl (i.e., a 1:1 ratio). This permitted the rapid cooling of the collected CT samples.
Generally, one should keep in mind that the isolated rat heart differs from in vivo physiological conditions, in that the interstitial fluid formation is most likely less than in the saline-perfused heart. The CT formed by the isolated heart may therefore not entirely mimic the true composition of the in vivo interstitial fluid. Additionally, the present setup does not permit the full exclusion of potential contamination with venous effluent perfusate. However, since the venous outlet is located at the cardiac base (the lowest level of the upright heart), we do not believe that contamination contributes to the collection process.
The present protocol describes a unique method to sample cardiac interstitial fluid, which contains a multitude of metabolites and proteins released into the interstitial fluid by cardiomyocytes and non-cardiac cells, such as immune cells, endothelial cells, vascular smooth muscle cells, fibroblasts, and pericytes. Cardiac interstitial transudate is formed as the result of fluid transport across the endothelial barrier10, together with a small fraction of lymphatic fluid. It contains a mixture of cardiac metabolites7, soluble factors in the interstitial space, and secretomes of cardiac and/or non-cardiac cells9. Therefore, several cell types contribute to the formation of CT. In addition, there are several factors that affect the rate of formation. First, oncotic pressure seems to be a major determinant that regulates trans-capillary fluid movement, since the increase of oncotic pressure by adding dextran or albumin to the perfusion medium significantly reduced the formation of CT9,10. Second, an increase in vascular permeability during hypoxia9,16, including MI, augments the extravasation of the perfusate and thus the formation of CT. Therefore, in future studies, the increase of oncotic pressure may be a suitable means to minimize the volume of CT, thereby enriching the targeted molecules of interest.

Heart failure (HF) is the leading cause of death in humans worldwide1. HF often occurs because of myocarditis, ischemic insults to the myocardium, and left ventricular remodeling, leading to the progressive deterioration of cardiac contractile function and patients&#39; quality of life. Although advances in cardiology and cardiac surgery have remarkably lowered HF mortality, they merely serve as transient &#34;delayers&#34; of an inevitably progressive disease process that carries signi&#64257;cant morbidity. Therefore, the current lack of effective treatment underscores the need to identify novel molecular targets that can prevent or even reverse HF. This includes changes in the extracellular matrix, uncontrolled cardiac immune response, and interactions between cardiac and non-cardiac cells2.
It is important to recognize that the microenvironment that cardiac cells are exposed to directly shapes the immune and regenerative response of the injured heart. In the isolated, saline-perfused heart, CT is generated on the heart surface in the form of small droplets that are derived from the interstitial fluid space (i.e., microenvironment), both under physiological and pathophysiological conditions3,4,5. Therefore, analysis of the CT (i.e., interstitial fluid) may help to identify factors that regulate cardiac metabolism and contractile function6 or influence immune cell functions after migration into the injured heart. Potentially, this may lead to the development of novel therapeutic strategies for the treatment of HF.
The collection of CT from murine hearts is technically challenging. In regular Langendorff-perfused hearts, the exclusive collection of CT is difficult because the mixture of the CT with coronary venous effluent perfusate unpredictably dilutes any concentration of metabolites/enzymes released from the interstitial space. One possible strategy to overcome this limitation is to exclude the venous effluent by cannulating the pulmonary and simultaneously ligating the pulmonary vein7. However, this method faces difficulties associated with the cannulation and ligation of the pulmonary artery and vein, causing potential leakage of venous effluent into the cardiac transudate. The concept of using a reverse heart model was first introduced by the group of Kammermeier, who inverted the isolated, perfused heart into an upside-down position and placed a thin latex cap on the epicardial surface to continuously sample CT without the contamination of venous effluent8,9. Using this procedure, CT was shown to provide a very sensitive measure of the metabolites released from the heart9, the capillary transfer of fatty acids8, and viral particles10.
More recently, paracrine factors that may regulate the local immune response and augment cardiac angiogenesis11 have been implicated in the beneficial effects of stem cell-based therapy for heart disease. The analysis of CT in the reversed heart may help to chemically identify these individual paracrine factors. In addition, CT may help to identify the factors involved in the in vivo activation of immune cells in the heart.
The detailed description of CT collection from the heart surface, provided here, is experimentally useful for researchers studying the interplay of immune cells, fibroblasts, endothelial cells, and cardiomyocytes in relation to overall cardiac function. As mentioned above, the interstitial fluid carries the information for cell-to-cell communication within the heart, which can conveniently be assessed by the collection of CT. The detailed technical description, including a video protocol of how to collect CT from the reversed heart, should facilitate the future application of this unique technique.

the inverted heart model for interstitial transudate collection from the isolated rat heart

The present protocol describes a unique approach that enables the collection of cardiac transudate (CT) from the isolated, saline-perfused rat heart. After isolation and retrograde perfusion of the heart according to the Langendorff technique, the heart is inverted into an upside-down position and is mechanically stabilized by a balloon catheter inserted into the left ventricle. Then, a thin latex cap &#8211; previously cast to match the average size of the rat heart &#8211; is placed over the epicardial surface. The outlet of the latex cap is connected to silicon tubing, with the distal opening 10 cm below the base level of the heart, creating slight suction. CT continuously produced on the epicardial surface is collected in ice-cooled vials for further analysis. The rate of CT formation ranged from 17 to 147 &#181;L/min (n = 14) in control and infarcted hearts, which represents 0.1-1% of the coronary venous effluent perfusate. Proteomic analysis and high performance liquid chromatography (HPLC) revealed that the collected CT contains a wide spectrum of proteins and purinergic metabolites.

The reversed-heart model enables the collection of cardiac interstitial transudate in an isolated, retro-perfused rat heart (Figure 1A-C). When perfused at a constant pressure of 100 cmH2O, the rate of interstitial fluid formation ranged between 17 and 147 &#181;L/min, amounting to 0.1-1% of the coronary venous effluent in the isolated heart.
The protein content of the CT, as measured with the bicinchoninic acid (BCA) assay, was found to be 1.08 &#177; 0.40 mg/mL (n = 6). One-dimensional gel electrophoresis analysis (SDS-PAGE) revealed a wide spectrum of proteins present in the cardiac transudate (Figure 2A). Two-dimensional fluorescence difference gel electrophoresis (2D-DIGE) was performed on CT from the heart subjected to 50 min of ischemia/reperfusion. As shown in Figure 2B, several proteins were found to be upregulated in the CT of ischemic hearts. Among the proteins identified, 70.1% were extracellular matrix proteins, 4.6% were cellular membrane proteins, 17.2% were cytoplasmic proteins, and 2.3% were nuclear proteins (Table 1).
Purines have long been considered pivotal signaling molecules that regulate the cardiac immune response, vasomotor tone, and cardiac function, particularly after ischemic injury. The collection of CT permits the measurement of a variety of metabolites present in the cardiac interstitial fluid under pathophysiological conditions, such as MI. As shown in Figure 3, the concentration of AMP, GMP, NADP, adenosine, hypoxanthine, and uric acid measured by HPLC where higher in the ischemic heart, which is similar to results previously reported using other methods16,17.

An Isolated Working Heart System for Large Animal Models

Since its introduction in the late 19th century, the Langendorff isolated heart perfusion apparatus, and the subsequent development of the working heart model, have been invaluable tools for studying cardiovascular function and disease1-15. Although the Langendorff heart preparation can be used for any mammalian heart, most studies involving this apparatus use small animal models (e.g., mouse, rat, and rabbit) due to the increased complexity of systems for larger mammals1,3,11. One major difficulty is ensuring a constant coronary perfusion pressure over a range of different heart sizes &#8211; a key component of any experiment utilizing this device1,11. By replacing the classic hydrostatic afterload column with a centrifugal pump, the Langendorff working heart apparatus described below allows for easy adjustment and tight regulation of perfusion pressures, meaning the same set-up can be used for various species or heart sizes. Furthermore, this configuration can also seamlessly switch between constant pressure or constant flow during reperfusion, depending on the user&#8217;s preferences. The open nature of this setup, despite making temperature regulation more difficult than other designs, allows for easy collection of effluent and ventricular pressure-volume data.

Most studies involving the Langendorff apparatus use small animal models due to the increased complexity of systems for larger mammals. We describe a Langendorff system for large animal models that allows for use across a range of species, including humans, and relatively easy data acquisition.

Isolation of Endocardial and Coronary Endothelial Cells from the Ventricular Free Wall of the Rat Heart

It has been shown that endocardial endothelial cells (EECs) and coronary endothelial cells (CECs) differ in origin, development, markers, and functions. Consequently, these two cell populations play unique roles in cardiac diseases. Current studies involving isolated endothelial cells investigate cell populations consisting of both EECs and CECs. This protocol outlines a method to independently isolate these two cell populations for cell-specific characterization. Following the collection of the left and right ventricular free wall, endothelial cells from the outer surface and inner surface are separately liberated using a digestion buffer solution. The sequential digestion of the outer surface and the inner endocardial layer retained separation of the two endothelial cell populations. The separate isolation of EECs and CECs is further verified through the identification of markers specific to each population. Based on previously published single cell RNA profiling in the mouse heart, the Npr3, Hapln1, and Cdh11 gene expression is unique to EECs; while Fabp4, Mgll, and Cd36 gene expression is unique to CECs. qPCR data revealed enriched expression of these characteristic markers in their respective samples, indicating successful EEC and CEC isolation, as well as maintenance of cell phenotype, enabling further cell-specific functional analysis.

An Immunological Model for Heterotopic Heart and Cardiac Muscle Cell Transplantation in Rats

Heterotopic heart transplantation in rats has been a commonly used model for diverse immunological studies for more than 50 years. Several modifications have been reported since the first description in 1964. After 30 years of performing heterotopic heart transplantation in rats, we have developed a simplified surgical approach, which can be easily taught and performed without further surgical training or background.
After dissection of the ascending aorta and the pulmonary artery and ligation of superior and inferior caval and pulmonary veins, the donor heart is harvested and subsequently perfused with ice-cold saline solution supplemented with heparin. After clamping and incising the recipient abdominal vessels, the donor ascending aorta and pulmonary artery are anastomosed to the recipient abdominal aorta and inferior vena cava, respectively, using continuous running sutures.
Depending on different donor-recipient combinations, this model allows analyses of either acute or chronic rejection of allografts. The immunological significance of this model is further enhanced by a novel approach of in-ear injection of vital cardiac muscle cells and subsequent analysis of draining cervical lymphatic tissue.

Optocardiography and Electrophysiology Studies of Ex Vivo Langendorff-perfused Hearts

Small animal models are most commonly used in cardiovascular research due to the availability of genetically modified species and lower cost compared to larger animals. Yet, larger mammals are better suited for translational research questions related to normal cardiac physiology, pathophysiology, and preclinical testing of therapeutic agents. To overcome the technical barriers associated with employing a larger animal model in cardiac research, we describe an approach to measure physiological parameters in an isolated, Langendorff-perfused piglet heart. This approach combines two powerful experimental tools to evaluate the state of the heart: electrophysiology (EP) study and simultaneous optical mapping of transmembrane voltage and intracellular calcium using parameter sensitive dyes (RH237, Rhod2-AM). The described methodologies are well suited for translational studies investigating the cardiac conduction system, alterations in action potential morphology, calcium handling, excitation-contraction coupling and the incidence of cardiac alternans or arrhythmias.

The objective of this study was to establish a method for investigating cardiac dynamics using a translational animal model. A Simplified Model for Heterotopic Heart Valve Transplantation in Rodents

There is an urgent clinical need for heart valve replacements that can grow in children. Heart valve transplantation is proposed as a new type of transplant with the potential to deliver durable heart valves capable of somatic growth with no requirement for anticoagulation. However, the immunobiology of heart valve transplants remains unexplored, highlighting the need for animal models to study this new type of transplant. Previous rat models for heterotopic aortic valve transplantation into the abdominal aorta have been described, though they are technically challenging and costly. For addressing this challenge, a renal subcapsular transplant model was developed in rodents as a practical and more straightforward method for studying heart valve transplant immunobiology. In this model, a single aortic valve leaflet is harvested and inserted into the renal subcapsular space. The kidney is easily accessible, and the transplanted tissue is securely contained in a subcapsular space that is well vascularized and can accommodate a variety of tissue sizes. Furthermore, because a single rat can provide three donor aortic leaflets and a single kidney can provide multiple sites for transplanted tissue, fewer rats are required for a given study. Here, the transplantation technique is described, providing a significant step forward in studying the transplant immunology of heart valve transplantation.

Rodent Working Heart Model for the Study of Myocardial Performance and Oxygen Consumption

Isolated working heart models have been used to understand the effects of loading conditions, heart rate and medications on myocardial performance in ways that cannot be accomplished in vivo. For example, inotropic medications commonly also affect preload and afterload, precluding load-independent assessments of their myocardial effects in vivo. Additionally, this model allows for sampling of coronary sinus effluent without contamination from systemic venous return, permitting assessment of myocardial oxygen consumption. Further, the advent of miniaturized pressure-volume catheters has allowed for the precise quantification of markers of both systolic and diastolic performance. We describe a model in which the left ventricle can be studied while performing both volume and pressure work under controlled conditions. 
In this technique, the heart and lungs of a Sprague-Dawley rat (weight 300-500 g) are removed en bloc under general anesthesia. The aorta is dissected free and cannulated for retrograde perfusion with oxygenated Krebs buffer. The pulmonary arteries and veins are ligated and the lungs removed from the preparation. The left atrium is then incised and cannulated using a separate venous cannula, attached to a preload block. Once this is determined to be leak-free, the left heart is loaded and retrograde perfusion stopped, creating the working heart model. The pulmonary artery is incised and cannulated for collection of coronary effluent and determination of myocardial oxygen consumption. A pressure-volume catheter is placed into the left ventricle either retrograde or through apical puncture. If desired, atrial pacing wires can be placed for more precise control of heart rate. This model allows for precise control of preload (using a left atrial pressure block), afterload (using an afterload block), heart rate (using pacing wires) and oxygen tension (using oxygen mixtures within the perfusate).

A Heterotopic Rat Heart Transplantation Model using Circulatory Death Donor Hearts

The objective of this protocol is to set up a rat heterotopic heart transplantation model with donation after circulatory death (DCD) donor hearts. There are two setups for this protocol: heart donor setup and recipient setup. In the heart donor setup, Sprague Dawley rats are anesthetized, endotracheally intubated, and ventilated. The right carotid artery is cannulated to deliver heparin and the paralytic agent vecuronium-bromide. The DCD process is initiated by terminating the ventilation. After 20 min, the heart is exposed and the aorta distal to the brachiocephalic branch is clamped. At 25 min from terminating the ventilator, ice-cold University of Wisconsin (UW) solution is perfused through the carotid catheter to flush the heart. The heart is procured by dividing the aorta, pulmonary artery, venae cavae, and pulmonary veins and stored in UW solution for implantation. In the recipient setup, the Lewis rat is anesthetized with isoflurane. Slow-release buprenorphine is administered subcutaneously to facilitate a smooth postoperative recovery. Through a midline abdominal incision, the infra-renal aorta and the inferior vena cava are isolated and clamped with an atraumatic vascular clamp. The donor heart aorta and pulmonary artery are sutured to the recipient abdominal aorta and vena cava, respectively, with a running 8-0 Prolene. The vascular clamp is removed to reperfuse the heart. The abdominal wall is closed and the rat is recovered. After a set interval (24 h to 2 weeks), the recipient rat is anesthetized, the transplanted heart is exposed, and a balloon-tip-catheter is inserted into the left ventricle via the apex to record developed pressure and dP/dt using a data acquisition system. The heart tissue is collected for histology, immunology, or molecular analysis. A successful DCD donor rat heart transplantation model will allow further studies on the cardioprotective approaches to improve heart transplantation outcomes from DCD donors.

A Model of Long-Term Ventricular Fibrillation in Isolated Rat Hearts

Ventricular fibrillation (VF) is a fatal arrhythmia with a high incidence in cardiac patients, but VF arrest under perfusion is a neglected method of intraoperative arrest in the field of cardiac surgery. With recent advances in cardiac surgery, the demand for prolonged VF studies under perfusion has increased. However, the field lacks simple, reliable, and reproducible animal models of chronic ventricular fibrillation. This protocol induces long-term VF through alternating current (AC) electrical stimulation of the epicardium. Different conditions were used to induce VF, including continuous stimulation with a low or high voltage to induce long-term VF and stimulation for 5 min with a low or high voltage to induce spontaneous long-term VF. The success rates of the different conditions, as well as the rates of myocardial injury and recovery of cardiac function, were compared. The results showed that continuous low-voltage stimulation induced long-term VF and that 5 min of low-voltage stimulation induced spontaneous long-term VF with mild myocardial injury and a high rate of recovery of cardiac function. However, the low-voltage, continuously stimulated long-term VF model had a higher success rate. High-voltage stimulation provided a higher rate of VF induction but showed a low defibrillation success rate, poor recovery of cardiac function, and severe myocardial injury. On the basis of these results, continuous low-voltage epicardial AC stimulation is recommended for its high success rate, stability, reliability, reproducibility, low impact on cardiac function, and mild myocardial injury.

Rat Model of Normothermic Ex-Situ Perfused Heterotopic Heart Transplantation

Heart transplantation is the most effective therapy for end-stage heart failure. Despite the improvements in therapeutic approaches and interventions, the number of heart failure patients waiting for transplantation is still increasing. The normothermic ex situ preservation technique has been established as a comparable method to the conventional static cold storage technique. The main advantage of this technique is that donor hearts can be preserved for up to 12 h in a physiologic condition. Moreover, this technique allows resuscitation of the donor hearts after circulatory death and applies required pharmacologic interventions to improve donor function after implantation. Numerous animal models have been established to improve normothermic ex situ preservation techniques and eliminate preservation-related complications. Although large animal models are easy to handle compared to small animal models, it is costly and challenging. We present a rat model of normothermic ex situ donor heart preservation followed by heterotopic abdominal transplantation. This model is relatively cheap and can be accomplished by a single experimenter.

Over the past 50 years, many researchers have reported heterotopic abdominal heart transplantation in mice and rats, with some variations in the surgical technique. Modifying the transplantation procedure to strengthen the myocardial protection could prolong the ischemia time while preserving the donor&#39;s cardiac function. This technique&#39;s key points are as follows: transecting the donor&#39;s abdominal aorta before harvesting to unload the donor&#39;s heart; perfusing the donor&#39;s coronary arteries with a cold cardioplegic solution; and topical cooling of the donor&#39;s heart during the anastomosis procedure. Consequently, since this procedure prolongs the acceptable ischemia time, beginners can easily perform it and achieve a high success rate.
Moreover, a new aortic regurgitation (AR) model was established in this work using a technique different from the existing one,&#160;which is created by inserting a catheter from the right carotid artery and puncturing the native aortic valve under continuous echocardiographic guidance. A heterotopic abdominal heart transplantation was performed using the novel AR model. In the protocol, after the donor&#39;s heart is harvested, a stiff guidewire is inserted into the donor&#39;s brachiocephalic artery and advanced toward the aortic root. The aortic valve is punctured by pushing the guidewire further even after the resistance is felt, thus inducing AR. It is easier to damage the aortic valve using this method than with the procedure described in the conventional AR model. Additionally, this novel AR model does not contribute to the recipient&#39;s circulation; therefore, this method is expected to produce a more severe AR model than the conventional procedure.

This study demonstrates a reproducible heterotopic abdominal heart transplantation technique in rats that beginners can learn and perform. Additionally, a novel aortic regurgitation model in rats is generated by performing heterotopic abdominal heart transplantation and damaging the donor's aortic valve using a guidewire after harvesting.
